Request For Proposal
Notice is hereby given that Carnation Cable Channel 11 will receive proposals for studio cameras , lenses, viewfinders, quick release plates, cables and installation to Alliance High School Broadcast Arts studio at 400 Glamorgan
Street, Alliance, Ohio, 44601.
Proposals are due June 14, 2013, 12:00PM EST (local time)Proposals shall be properly and completely executed as indicated in the Instructions to Bidders, located on the district webpage at www.alliancecityschools.org. Late proposals will not be accepted.

Summer Meals
Spring Award Night
Dear Parent / Guardian,
The 2013 Spring Sports Award Night will be held on Tuesday, May 21, at 7:00 PM, in the Alliance High School auditorium, where all the spring sport athletes and their parents/guardians will gather for the presentation of some awards to be given out. The individual sports will be dismissed later to specific rooms for their own presentation of awards.
All athletes must attend this award night to finish the season in good standing with their specific sport. Any problems must be discussed in advance with me.

THIS IS IMPORTANT INFORMATION REGARDING THE FREE PHYSICALS FOR THE 2013/14 SCHOOL YEAR

STUDENT ATHLETES ONLY

ALLIANCE HIGH SCHOOL PHYSICALS WILL BE WED MAY 15TH AT THE HIGH SCHOOL
BEGINNING @ 11 AM
BLOOD PRESSURE, HEIGHT & WEIGHT WILL BE DONE IN THE AHS CAFTETERIA DURING
LUNCHES FOR ALLIANCE HIGH SCHOOL BEGINNING
MAY 9 THRU MAY 15th
ALLIANCE MIDDLE SCHOOL PHYSICALS WILL BE WED. MAY 8TH @ ALLIANCE MIDDLE
SCHOOL FROM 11AM TO 1 PM
BLOOD PRESSURE, HEIGHT & WEIGHT FOR ALLIANCE MIDDLE SCHOOL WILL TAKE
PLACE DURING LUNCHES ON WED
MAY 2 THRU MAY 8TH
YOU MUST HAVE YOUR PHYSICAL FORM FILLED OUT COMPLETELY AND SIGNED BY
A PARENT OR GUARDIAN BEFORE YOU CAN GET YOUR HEIGHT, WEIGHT & BLOOD
PRESSURE DONE.
THE PHYSICAL FORM WILL THEN BE COLLECTED BY THE NURSES AND RETURNED TO
YOU WHEN YOU COME TO GET YOUR FREE PHYSICAL AT YOUR RESPECTIVE SCHOOL
AFTER YOUR PHYSICAL IS COMPLETED, WE WILL COLLECT THEM ALONG WITH YOUR
COMPLETED INSURANCE PACKET & CONCUSSION FORM.

BLOOD PRESSURE, HEIGHT & WEIGHT MUST BE DONE IN ADVANCE OF THE PHYSICALS.

IF YOU ARE PLANNING ON PLAYING A SPORT ALONGWITH THE NEW CONCUSSION FORM.
DUE TO A CHANGE, YOU WILL ONLY HAVE TO FILL YOUR INSURANCE PACKET OUT
ONCE FOR THE YEAR, BUT ARE RESPONSIBLE TO PROVIDE THE ATHLETIC DEPT
WITH ANY CHANGES CONCERNING YOUR EMERGENCY MEDICAL FORM OR INSURANCE.
